I will ask you the reason you would like to see a nutritionist.

  • New medical diagnosis
  • Struggling with lab levels (high blood sugar, cholesterol, etc)
  • Wanting to improve health
  • Other

If you have insurance, I will check your insurance information with your approval, I will need:

  • Member identification number on your insurance card
  • Date of birth
  • An appointment will be scheduled (Please allow 90 minutes for a first visit), once coverage information is obtained

BEFORE YOUR FIRST VISIT


  • You will receive an email from me
  • The email will have forms for you to complete
  • Complete the forms and click the submit button

AT YOUR FIRST MEETING


  • I will review the completed forms
  • Assess your eating habits and lifestyle
  • We determine what will work for you to help you reach your goals

AFTER YOUR FIRST MEETING


  • You will have nutritional interventions to implement
  • We will reschedule should you choose to and change or add interventions depending on your situation
  • Your nutritional situation may take one visit or more, this is dependent on the individual and insurance coverage

I accept Blue Cross and Cigna insurance.  Discounted rates are available for other insurances and private pay.
